Defeat Duchenne Canada is in the process of developing its next strategic plan, and we are looking for feedback from our trusted and valued community of families, donors, partners, volunteers, supporters and friends.

In partnership with SWON Public Affairs, we’re offering all survey respondents a chance to WIN a $200 Amazon gift card! The survey will close on Friday, June 23, at midnight, the draw will take place on Monday, June 26, and the winner will be contacted via email. To enter the draw, please include your email address at the end of the survey (your email address will not be connected to your survey responses).

Please note:

• Your feedback is completely anonymous, and results will only be reported collectively.
• This survey is not a ‘test’ of your knowledge. We want your immediate ‘off the top’ answers as you feel and know now. If you feel you don’t have enough knowledge to answer a question, that’s fine; indicate that in your answer and move on.
• We ask that you complete the survey in one sitting. If there are others in your household connected to Defeat Duchenne Canada, we encourage them to complete their own survey. If they use the same computer, please ensure each person completes and submits their survey before the next person begins.
• The survey should take no more than 20- 25 minutes and is best completed on a desktop computer, laptop, or tablet.
